This notice of intent is neither a formal solicitation nor a request for competitive proposals. No solicitation document is available and telephone requests will not be honored. No award will be made based on unsolicited quotations or offers received in response to this notice. Any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the Government. The intent of this synopsis is for informational purposes only. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete this action based on this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Scope of Work
The services to be provided by the contractor under this agreement shall consist of a full contract that provides for all replacement/exchange service parts, components needed for repairs, and preventive maintenance for the Long Beach VA Healthcare System owned Hologic Selenia Mammo Unit (EE95218).
PREVENTIVE MAINTENANCE INSPECTION:
Preventive Maintenance Inspections (PMI) is completed annually, semi-annually, or quarterly as prescribed by manufacturer.

Specifications and Requirements: All work performed shall be accomplished in accordance with manufacturer’s specifications including, but not limited to adjustments, calibrations, cleaning,
lubrication, testing, disassembly, check-out, replacement of worn or defective parts, etc. required to keep the equipment in optimum operating condition. All equipment must be maintained in such condition which shall satisfy the requirements of the Department of Health, Education and Welfare, National Fire Protection Association (NFPA), Joint Commission (JC), Association for the Advancement of Medical Instrumentation (AAMI) and the State of California. Vendor shall apply VA supplied PMI labels (when available) to all devices listed in this contract.

At a minimum, PMI are to include:
– Wear, misalignment, maladjustment, damage, or other malfunctions with respect to the
manufacturer’s specifications
– Testing of electrical components for proper voltage and grounding for safety and fire hazard
– Cleaning and lubrication of equipment as required
– Repair or replacement of all defective components necessary to conform to instrument
specifications
– Calibration or standardization of instrument as required.
– Testing for proper operation of the entire system and subsystems
– Replacement of defective parts
– Documentation on proper function and performance of instrument
– Software updates, if applicable
– If a device cannot be located, Contractor must report to Contracting Officer
Representative (COR) for additional attempts to locate device for service

Services are to be scheduled at least three (3) days in advance with the Contracting Officer
Representative (COR) and services are to be completed within one (1) day. If applicable, software updates are included for the general operation of the equipment.

Period of Agreement: Unless otherwise specified, this contract will be for the period or performance 4/24/21 through 4/23/22.

Hours of Coverage:
Normal hours of coverage will be Monday through Friday from 8:00am to 5:00pm, excluding federal holidays.
All service/repairs will be performed during stipulated hours of coverage unless requested or approved by Biomedical Engineering.

Services to be Provided:
Telephone Technical Support: Telephone assistance will be provided 24/7, 365 days/year.
Response Times: Contractor will acknowledge requests by telephone for service within 2 hours.
The contractor will always contact the Biomedical Engineering Section, (562) 826-5977 before and after arrival at the site.

Documentation: Contractor will provide individual written service reports which describe services performed on the equipment (in sufficient detail to be acceptable to the field inspectors of The Joint Commission). The following information must be provided in each service report:
1) Date of service
2) Summary of problem and action taken
3) Parts replaced
4) Labor hours
5) Travel time
6) Equipment serviced to include description, equipment ID #, serial number, etc.
7) Purchase order number
Note: To ensure proper equipment history documentation, the contractor will forward all preventive maintenance records within 5 working days of completion to Biomedical Engineering.
Note: This report must be approved and signed by Biomedical Engineering personnel or their representative. A copy of each report will be left with Biomedical Engineering. In the event that Biomedical Engineering is not available to receive the service report (outside normal working hours), the service report will be left with the supervising official of the equipment that was serviced; the supervising official will forward the service report to Biomedical Engineering (138B) the next working day.
Qualifications: Contractor shall be engaged full-time in the maintenance and repair of the equipment listed in Section I.
Contractor shall have access to complete service manuals
Contractor shall have access to software diagnostic keys
Contractor shall, without exception, provide all hand tools, diagnostic test equipment, ladders, and anything else necessary to perform the work
