This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. 2. Solicitation number 36C25921Q0332 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-02 Effective November 23,2020. 3. This acquisition is a 100% Total SDVOSB (Service-Disabled Veteran Owned Small Business) Set-Aside. The North American Industry Classification System (NAICS) code for this procurement is 621399 Offices of All Other Miscellaneous Health Practitioners, with a small business size standard of $8 Million. 4. Description of services: In accordance with the Statement of Work (SOW) and price schedule, the Contractor shall price covid enhanced screening services for the VHA Office Of Community Care located at 3773 Cherry Creek Drive North, Suite 470 Denver, CO 80209. 6. Period of Performance (POP): The period of performance shall consist of one (1) twelve (12) month base-year period and two (2) twelve (12) month option year periods. 7.
